Health Home Coordinator
serving the child population.


Description
Behavioral Health Home - Become part of a Team staffed with a Clinical Supervisor, Nurse Care Manager and Peer Support Specialists working with children in the management of physical and behavioral health needs.

As part of the Behavioral Health Home model, the Health Home Coordinator is responsible for assisting children with serious emotional disturbance and their families by providing rehabilitative and treatment services as well as environmental supports that promote the integration of the person into the community, sustains the person in his/her current living situation or another living situation of the family's choice, and enhances the quality of his/her life.

Some responsibilities of this position are to: Collaboration with primary care providers Find, link and coordinate needed resources Monitor effectiveness of current services & support Advocate for change and additional resources when needed Provide regular contact, monitor the recovery process, and ensure consumer participation and involvement in setting and reaching goals Specific activities might include providing assistance with finding housing, medical care, or transportation and meeting safety needs. Requirements
Minimum of Bachelor's degree in related field (or unrelated field with a minimum of one year related experience in the field)--OQMHP eligibility is required. Valid driver's license and computer proficiency are also required. Duties require knowledge of and experience in working with children with special needs and the ability to adapt to the individual needs of a child/family.
