Easterseals Maine is seeking an organized, family-focused, and strengths-based Targeted Case Manager to join our team in Portland. You will work with youth and their families with a range of mental health diagnoses in greater Portland and surrounding areas. Responsibilities will include assessing needs, developing service plans, making referrals, and monitoring outcomes with clients and their families, their natural supports, and with other committed professionals who serve to reduce barriers and serve as "one stop" resource for coordinating services. This position requires a Bachelor's Degree in Social Work or a related field, two (2) years of human services experience and at least two (2) years of experience as a children's case manager. Candidate will have excellent time management, communication, computer, and family and client engagement skills. A reliable vehicle and insurance which meets our coverage requirements are also required. We do provide mileage reimbursement.
