Children’s Case Manager
Waban is seeking a skilled, motivated, and highly organized professional to join our growing Children’s Case Management program. A Children’s Case Manager assists parents of children from birth to 21 years of age who have Intellectual Disabilities, Mental Health Disorders, Autism Spectrum Disorders, or other developmental delays/disabilities. The Case Manager assists the family in accessing necessary services and supports for their children. With the family, the Case Manager identifies the child’s strengths and needs, develops and coordinates a plan of care tailored to the child, connects families to services and supports, and helps ensure that the child’s needs are being met.
Case Managers work closely with a multidisciplinary team of professionals from schools, child welfare, and other agencies, and other support people chosen by the parent and child. The Case Manager conducts intakes, completes comprehensive assessments of the child’s strength and needs, coordinates and advocates for services identified in the plan, monitors the child’s progress, and evaluates the effectiveness of services.
*This position requires travel in the community*
Must have a reliable vehicle with proper insurance, a valid driver’s license from state of residency and a clean driving record.
Must be proficient in speaking, reading and writing English with strong interpersonal communication skills.
Must be able to complete agency and State mandated training programs with proficiency.
Must be able to use a computer to enter time & attendance, document all aspects of service delivery, complete trainings, and communicate effectively with supervisor, co-workers, families, and other professionals.
Bachelor’s Degree in Social Work or related field in accordance with Mainecare Targeted Case Management Policy and DHHS contract.
Minimum of one year experience in human services field.
