Occupational Therapist - Contracted

Requirements: 

• Hold a OT master’s degree in occupational therapy
• Hold a valid state of Maine state license to practice
• Hold a valid state of Maine criminal history records check approval 

Responsibilities:

• Provide in person occupational therapy to students with occupational therapy needs as written in their Individual Education Plan (IEP) 
• Conduct in person occupational therapy evaluations, re-evaluations, or screenings, as requested by the IEP team
• Provide written evaluation reports
• Consult with teachers and occupational therapist assistant to assist with the evaluation and potential solutions to treatment and therapy problems for students and for adherence to the Individual Education Plan (IEP)
• Attend IEP team meetings as requested by schools (can attend via zoom)
• Provide IEP goals to school case-manager as needed for IEP
• Review and verify schedules weekly to ensure all scheduled visits are accurate
• Provide progress reports to the special education case-manager for students, on a quarterly or trimester basis, for report cards
• Maintain therapy progress notes 
• Complete school service logs
• Prepare therapy materials
• Travel between sites, as assigned, to perform therapeutic activities
• Complete business service logs
• Clock in and out 
• Keep mileage logs for reimbursement
